The Cathedral is the seat of the Bishop for Birmingham, the Right Revd David Urquhart. Its chief executive is the Acting Dean, Revd Canon Peter Howell-Jones, who chairs the Cathedral Chapter, or governing body. The Cathedral is at the heart of the Diocese of Birmingham.
As well as being the central church of the Diocese, it is also a parish church, whose geographical area includes the Council House and Town Hall, many major business and finance institutions, theatres and hospitals, retail outlets both large and small, restaurants, hotels and pubs, and an increasing number of residential properties. Nearly all the major diocesan services take place here.

Birmingham Diocese covers the City of Birmingham and the District of Solihull, as well as parts of Warley, Warwickshire and Worcestershire. The Cathedral benefits from the work of the Friends of Birmingham Cathedral, a group of supporters from all over the world. The Friends hold regular meetings and raise money for specific aspects of the Cathedrals life and work. The Chair of the Friends is Sir Ian Byatt.
